OSHA has again revised its Recordkeeping rule (29 CFR part 1904) effective January 1, 2004. One of the revisions includes a column for recording hearing loss on the 300 Log. Required forms include:
OSHA Form 300, Log of Work Related Injuries and Illnesses
OSHA Form 300A, Summary of Work Related Injuries and Illnesses
OSHA Form 301, Injury and Illness Incident Report
The most recently revised forms (2004 Forms) will be used to record information for 2004 and forward. Injuries and illnesses for years prior to 2004 should continue to be recorded on the appropriate form for that year.
Copies of the new form are available below. Copies and instructions for use can also be obtained from OSHA’s web site at http://www.osha.gov or from the OSHA publication office at 202-693-1888. Assistance is also available from the Technical Resource Department of Employer’s Claim Management, Inc. (334-277-9395).
Note: All employers covered by the Occupational Safety and Health Act are covered by 29 CFR part 1904 regulations. However, most employers do not have to keep OSHA injury and illness records unless OSHA or the Bureau of Labor Statistics informs them in writing that they must keep records. For example, employers with 10 or fewer employees and business establishments in certain industry classifications are partially exempt from keeping OSHA injury and illness records.
